Summary
Lido's Community Staking Module is the permissionless tier of its operator set: anyone can run validators without being selected by the DAO, provided they post a stETH bond. The bond is the mechanism that makes an unvetted operator safe to include — penalties incurred by that operator's validators are taken from it once those validators leave the network.
On 2026-03-13 at 20:38 UTC, six validators belonging to one permissionless CSM node operator were slashed. Lido characterised the cause as an operator-side issue and did not publish a root cause or name the operator.
The penalties came to less than 0.047 ETH (~$100), with the total projected to remain under 1 ETH if nothing further surfaced. Lido stated that the operator's bond covers the penalties in full and that stakers were unaffected, noting that the figure is smaller than the daily variation in the protocol's rewards.
The contrast Lido drew is with 2023-10-11, when twenty validators run by Launchnodes were slashed over validator configuration problems — an event an order of magnitude larger, from the vetted operator set, before CSM existed.

What defenders observed
- The interesting number is not the penalty, it is who paid it. $100 of slashing is noise. That the $100 came out of the operator's bond rather than out of pooled staker rewards is the property the design exists to produce, and it is the property that should be verified — by reading the bond accounting, not the announcement.
- Permissionless operator sets shift the question from vetting to containment. A curated set spends its risk budget on selecting operators; an open set cannot, so it must cap what any single operator can cost everyone else. The per-operator bond is that cap, and its adequacy is testable in advance: bond size against the worst-case correlated slashing for the validators that operator runs.
- A slashing event with no attacker is still a security event. OAK records it because the failure mode, the blast-radius question and the control are identical whether the trigger is a misconfiguration or an adversary — and because the adversarial version of this class (deliberately triggering slashing conditions against someone else's validators) is only interesting if you know what the containment does under an accidental one.
- No root cause was published. Lido reported the slashing, the amount and the coverage, but not the fault. For a permissionless module that is a defensible operational choice and a gap in the public record: other CSM operators cannot learn from a configuration error they are not told about, which is exactly the population most likely to repeat it.

Discussion
T14's corpus entries skew toward the loud end of the Tactic: cascading restaking risk, LST depegs under constrained primitives, liveness-fault griefing. This one is at the other end deliberately. OAK's T14 coverage needs at least one case where the staking design's loss-containment mechanism was exercised and held, because every mitigation the corpus recommends for this Tactic — bonds, per-operator caps, correlated-slashing limits — is otherwise argued from theory and from incidents where the containment did not exist.
The structural comparison inside Lido's own history makes the point better than any cross-protocol one. In 2023 twenty validators from a vetted operator were slashed; the operator was known, selected and trusted, and the loss landed in the protocol's shared accounting. In 2026 six validators from an unvetted operator were slashed, and the loss landed on that operator's bond. The set that looks riskier on paper — anyone can join — is the one where the exposure was capped in advance, because it had to be.
For contributors documenting staking incidents: record which operator tier the fault came from (vetted, permissioned, permissionless), what bond or collateral stood behind it, and whose balance absorbed the penalty. Those three fields separate a staking design that distributes risk from one that merely distributes yield, and none of them appear in the loss figure.
